Glossary of terms and forms
TermsAML, Additional Maternity Leave: An additional 26 weeks of maternity leave that starts at the end of Ordinary Maternity Leave.
EDD, estimated due date: The date the baby is due, as given on form MAT B1
EWC, expected week of childbirth: The week the baby is due
HSE, Health and Safety Executive: The government organisation responsible for health and safety issues at work
MA, Maternity Allowance: Maternity pay paid by the Jobcentre Plus if you are not eligible for Statutory Maternity Pay
OH, occupational health: Clinicians who work in an occupational setting looking at the impact of work on health, and health at work
OML, Ordinary Maternity Leave: The first 26 weeks of maternity leave
Paid maternity leave: Maternity leave where you are paid either Statutory Maternity
Pay or contractual maternity pay
Qualifying period: The period of time used to assess your right to claim certain benefits
Statutory: Rights that apply by law
SMP, Statutory Maternity Pay: Maternity pay from your employer
SPP, Statutory Paternity Pay: Paternity pay for fathers and partners who will be responsible for the upbringing of the baby
SSP, Statutory Sick Pay: Sick pay from the employer
Unpaid maternity leave: Maternity leave without pay FormsMA1: A form from your local Benefits Agency/Jobcentre Plus to claim for Maternity Allowance (MA)
MAT B1: A maternity certificate from your GP or midwife confirming the due date of your baby. You must give this to your employer so you can claim leave and pay due to you
MED 3: A medical form from your GP or midwife outlining any medical issues related to your pregnancy that may affect you at work and which the employer must take into account in a risk assessment
FW8: A form from your midwife or GP entitling you to free prescriptions and dental treatment during pregnancy and for a year after the birth of your baby
SF100: A Sure Start Maternity Grant form for those in receipt of some benefits (such as jobseekers allowance). Available from your local Jobcentre plus.
SMP1: A form from your employer explaining why you are not eligible for Statutory Maternity Pay. Give this to the Benefits Agency/Jobcentre Plus with form MA1 to claim for Maternity Allowance (MA)
